Phase of scar formation
The course of renal inherent fibrosis is in a phase of scar formation when renal cells turn to fibroblasts owing to inflammation and effects of cell factors and growth factors. It has been discovered that when turn to fibroblasts, renal cells do not change because of inflammatory mediators and cell factors of primary disease, but depend on autonomic hyperplasia, synthesis, and cecretion. They cecrete collagens1,3,which are hard to degrade. These collagens can accelerate the synthesis of ECM abnormally and slower degradation of ECM. The disbalancement between synthesis and degradation causes formation of fibrous tissue that makes ECM to deposit and accumulate abnormally. Glomerulus's sclerosis, of renal tubular fibrosis, renal interstitial fibrosis, Reno vascular fibrosis, and lasting scar appears at last.In this period, effective nephrons disappeared gradually, and renal function fails progressively.This period is called end stage of renal failure or uremic stage.It is possible to block the course of renal failure in this phase, but it is very hard to repair renal tissue that has already been scars.
